The TRACTION trial was a multi-centre, registry-based, randomized, placebo-controlled, cluster-crossover trial conducted at 10 Canadian hospitals to evaluate whether routine tranexamic acid (TXA) use in patients undergoing major non-cardiac surgeries reduces red blood cell (RBC) transfusion while maintaining safety. Patients ≥18 years undergoing inpatient surgeries with ≥5% transfusion risk received either 1-gram TXA bolus (2 grams if >100 kg) at surgery start plus 1 gram before closure, or matching placebo, with hospitals switching treatments every 4 weeks in a blinded manner. Co-primary outcomes were transfusion rate and 90-day venous thromboembolism; secondary outcomes included transfusion volume, cardiovascular events, length of stay, ICU admission, and 90-day survival.
An International, multicenter, adaptive platform trial evaluating multiple interventions to improve outcomes in adults hospitalized with respiratory infections. The trial will be conducted at over 100 sites in Canada, the U.S., Mexico, Brazil, Colombia, and Australia. Currently, the trial is studying anticoagulation, corticosteroids, and antibiotic regimens in patients with community acquired pneumonia on the ward.
A National feasibility pilot trial randomized control trial (RCT) evaluating the use of therapeutic plasma exchange (TPE) in critically ill patients with septic shock. The trial is currently being conducted at twelve Canadian sites.
